FOR HIGH SCHOOL PLAYERS ONLY:

**Registration** High School Registration Packet drop off will be held on December 22nd and January 10th  at the High School from 6:30-7:00pm EST.  Registration Packet Drop off will be located at  the Front Doors to Main Street.   These doors are on the opposite side of the school from the gyms, facing the front parking lot.

Please Note – When your Physical Form (all six pages) have been completed, please have your player hand his/her Physical Form (all six pages) ONLY to the Coaches.   *Please use the Button labeled “Online Registration” to fill in the Registration page portion of the packet in lieu of turning the hard copy in.

**If you choose to register online please note you do NOT need to sign up for a US Lacrosse Membership to participate at the high school level.
